Transcript Requests

Current Students

Current Students can request official transcripts in the Guidance Office.  Two forms are utilized:

•  A transcript release form (parent signature required).  This gives the school permission to send your transcript to any institution or organization that makes a request. 
• A transcript request form indicates the number of official and unofficial transcripts you are requesting.  Students may pick up transcripts in the Guidance office on the next school day.  Students are responsible to address and affix postage to official transcripts for mailing.

During Remote Learning and when submitting applications online, requests can be made electronically by emailing your counselor. Please provide the email address for where you would like the transcript sent.

Former Students

Former students can request transcripts by contacting Lisa Smith at 724-439-5000 x1500, or email at lisa.smith@uasdraiders.org.
